All agencies sending recruits/students to the academy must complete a Notice of New Appointment (NONA PTC-3) Form. This includes Basic Courses, SLEO I/II, SLEO II to Basic and a Request for Waiver of training (PTC-15) for certain trainings such as HLEO courses. Agencies are required to send the PTC-3 or PTC-15’s to the PTC in accordance with N.J.A.C Title 13. These forms must be completed 30-days in advance of the course. The forms can be found in your agency’s training manual, but can also be printed out here:

Agencies that do not submit these forms for their officers are subject to removal from the class.
